Jeff,
I have not totally followed this whole thread, but I thought I should
report that on a production linux box that I help support, we get random
failures of mapcache that require an apache restart. I have not been
able to identify this problem and put a temporary hack of a cronjob to
restart apache periodically but we still have the problem just not as often.
Running: Ubuntu 14.04.4 LTS
libapache2-mod-mapcache:amd64/trusty 1.2.1-2 uptodate
libmapcache1:amd64/trusty 1.2.1-2 uptodate
So there may be an underlying mapcache issue that might not be related
to MS4W.
